My Fish Is Swimming Upside Down: Swim Bladder Disorder Explained
A fish floating upside down looks like a fish about to die. Usually it isn’t. Swim bladder disorder is common, often reversible, and frequently caused by something as mundane as how you feed.
The important thing to understand up front: like fin rot, swim bladder disorder is a symptom. Several unrelated problems produce it, and they have different fixes. Working out which one you have is the whole job.
What the swim bladder does
The swim bladder is a gas-filled organ that controls buoyancy. When it’s working, a fish holds position without effort. When it’s compressed, over-inflated, or damaged, the fish loses that control and floats, sinks, or tilts.
How your fish fails tells you a lot:
- Floating at the surface, often upside down or on its side — usually a compressed or over-inflated bladder from digestive swelling. The most common presentation and the most treatable.
- Sinking to the bottom, unable to rise — often a deflated bladder, or a fish too weak to compensate. More concerning.
- Head-down or tail-down tilting — partial dysfunction, frequently digestive.
- Spiralling or rolling uncontrollably — this points to a neurological or bacterial cause rather than a mechanical one, and it’s the most serious version.
The four causes, in order of likelihood
1. Constipation and overfeeding — the usual answer
An impacted digestive tract presses on the swim bladder. This is by far the most common cause, especially in fancy goldfish and bettas, and especially in fish fed dry flakes or pellets as their only diet.
Dry food is the specific culprit. Flakes and pellets expand as they absorb water, and a fish that gulps them at the surface swallows air along the way. Both make the problem worse.
2. Body shape (fancy goldfish)
Fancy goldfish — orandas, ryukins, fantails, ranchus — have been bred for compressed, rounded bodies that physically crowd the swim bladder and kink the intestine. Some individuals will have recurring buoyancy problems their whole lives. This is managed, not cured, and it’s worth knowing before you pursue increasingly aggressive treatment.
3. Water quality and temperature
Cold water slows digestion dramatically, which turns a minor food issue into a blockage. Ammonia and nitrite exposure damages organ function generally. Test before assuming it’s diet.
4. Infection or injury
Bacterial infection, physical trauma, or parasites can damage the bladder directly. Suspect this if buoyancy problems come with clamped fins, loss of appetite, pineconing scales, red streaking, or if the fish is rolling rather than simply floating.
The treatment that works for most cases
If your fish is floating or tilting, is otherwise alert, and is still interested in food, work through this before anything else.
Step 1 — Fast the fish for 3 days
No food at all. This is safe for any healthy adult fish and it clears the digestive tract. Most people find this uncomfortable and cheat; don’t. Three days of fasting resolves a large share of cases on its own.
Step 2 — Raise the temperature slightly
For tropical fish, move toward the upper end of the species range (around 80°F) to speed digestion. For goldfish, 68–72°F is enough — they don’t want tropical temperatures. Change temperature gradually, no more than 2°F per hour.
Step 3 — Check your water
Test ammonia, nitrite, and nitrate. Do a 25% water change. If ammonia or nitrite is detectable, that’s a contributing cause and needs fixing regardless.
Step 4 — Feed a deshelled cooked pea
After the fast, offer one small piece of cooked, skinned green pea. Peas provide fibre that helps clear the tract. Cook until soft, remove the outer skin, and break off a piece roughly the size of the fish’s eye. One piece is plenty.
Step 5 — Change how you feed permanently
This is the part that prevents recurrence:
- Soak dry food before feeding so it expands in the cup rather than in the fish.
- Use sinking food for species prone to this, so they aren’t gulping air at the surface.
- Feed less. An adult fish needs what it can eat in about 30 seconds, once or twice a day.
- Add variety — frozen daphnia in particular acts as a mild laxative for fish.
- Keep one fasting day per week for goldfish and bettas.
When it isn’t constipation
Escalate if the fish is rolling or spiralling rather than floating steadily, if it has stopped eating entirely, if scales are pineconing outward (that’s dropsy — a different and much more serious problem), if there’s red streaking or visible injury, or if three days of fasting produces no change at all.
Those cases suggest infection or organ damage, and they need an antibacterial treatment in a hospital tank rather than a dietary fix.
Living with a chronic case
Some fish — particularly fancy goldfish — never fully resolve. A fish that floats but still eats, responds, and shows no distress can live a long and reasonable life. Practical accommodations:
- Lower the water level so reaching the surface is less work.
- Make sure any part of the body sitting above the waterline stays wet, or it will develop sores.
- Use sinking food if the fish sits on the bottom, floating food if it can’t descend.
- Remove sharp decor — a fish with poor control will bump into things.
Judge quality of life on eating, responsiveness, and absence of distress rather than on posture alone. A permanently tilted fish that eats enthusiastically is doing better than it looks.
