Dropsy in Fish: Pineconing Scales, Causes and Realistic Odds
Dropsy is the diagnosis most keepers dread, and the honest framing matters here: it is not a disease. It is a symptom — fluid accumulating in the body cavity, usually because the kidneys or another organ system is failing. The underlying cause is what kills, and by the time scales are visibly raised the internal damage is often advanced.
Survival rates are genuinely low. That is not a reason to do nothing, but it is worth knowing before you spend two weeks and a lot of money.
Recognising it
The definitive sign is pineconing: the abdomen swells until the scales are pushed outward from the body, so that viewed from directly above the fish looks like a pinecone. The top-down view is the diagnostic — from the side, a swollen fish just looks fat.
Alongside it you may see bulging eyes, pale or stringy waste, loss of appetite, lethargy and resting on the bottom, clamped fins, rapid breathing, and sometimes red streaking or ulcers.
What it is not
- Constipation or overfeeding — swelling without raised scales. The single most common false alarm.
- A gravid female — swelling is smooth, scales lie flat, and the fish behaves normally.
- A tumour — usually asymmetrical, one-sided, developing slowly.
- Swim bladder disorder — buoyancy problems without pineconing.
If the scales are flat, it is not dropsy, and you should be looking at the constipation and swim bladder path instead.
What is actually happening
Healthy fish constantly regulate the movement of water and salts across their tissues. When the kidneys or liver fail, that regulation breaks down and fluid accumulates in the body cavity faster than it can be expelled. The pressure pushes the scales outward.
The underlying causes are usually one of: bacterial infection (frequently Aeromonas), organ failure from age or chronic stress, parasitic damage, or long-term exposure to poor water quality. Almost every case traces back to something that compromised the fish over weeks or months — chronically elevated ammonia or nitrate, an unheated tank, or persistent stress.
Dropsy is not meaningfully contagious in itself. But whatever caused it — bad water, an infection, chronic stress — may be affecting your other fish too. Check the tank, not just the patient.
Treatment
Act immediately; the odds fall every day.
1. Hospital tank
Move the fish to a separate tank so you can medicate accurately without destroying your display tank’s biological filter. Match temperature and parameters, and keep it bare-bottomed for easy cleaning.
2. Epsom salt, not aquarium salt
This distinction matters and is frequently got wrong. Epsom salt is magnesium sulfate and it helps draw retained fluid out of the tissues by osmosis. Aquarium salt is sodium chloride and does not do this. Use roughly 1–2 teaspoons of Epsom salt per 10 gallons in the hospital tank.
3. Antibacterial medication
Since bacterial infection is the most common underlying cause, a broad-spectrum antibacterial treatment is warranted. Medicated food is more effective than water treatment if the fish is still eating, because the infection is internal — water-column medication struggles to reach it. Complete the full course.
4. Pristine water
Daily water changes in the hospital tank, re-dosing Epsom salt and medication for the volume replaced. A failing organ system needs every reduction in load it can get.
5. Feed carefully
Offer high-quality food if the fish is eating. Do not force it if not.
The honest prognosis
Caught very early — slight swelling, scales just beginning to lift, fish still eating and active — recovery happens and is worth pursuing. Once pineconing is pronounced, the fish has stopped eating, and it is lying on the bottom, treatment rarely succeeds.
At that point the kinder question is about suffering rather than cure. A fish that has stopped eating, cannot maintain position, and is visibly distressed is not going to recover, and prolonging it serves nobody. Clove oil is the standard humane method if you reach that decision.
Prevention
Dropsy is largely a disease of chronic poor conditions. Keep ammonia and nitrite at zero and nitrate under 40 ppm, hold a stable correct temperature, do not overstock, feed a varied quality diet, and quarantine new fish. Most cases are the end point of months of small problems rather than a sudden event — which means the prevention is just consistent husbandry, done unglamorously and continuously.
