White Spots on Fish: Is It Ich? Identification and Treatment

You just noticed small white dots on your fish — on the fins, along the body, maybe scattered like someone shook a salt shaker over the tank. Almost everyone’s first guess is ich, and most of the time that guess is right. But not always, and the treatments for the lookalikes are different enough that getting it wrong costs you fish.

What ich looks like — specifically

Ich is caused by Ichthyophthirius multifiliis, a parasite that burrows under the fish’s outer layer of skin and forms a visible cyst. The classic description is “grains of salt,” and that’s accurate in a way worth being precise about:

  • Discrete and raised. Individual dots with clear edges, sitting slightly proud of the skin. Not a film, not a haze.
  • Roughly uniform in size — about 0.5 to 1 mm, like fine table salt.
  • Scattered, not clustered. Spots appear across the body and fins somewhat randomly rather than concentrated in one patch.
  • White or very pale, never fuzzy. If it has texture — cottony, threadlike, hairy — it isn’t ich.

Behaviour usually shows up before the spots do. Fish scrape against decor and substrate (flashing), clamp their fins, breathe faster, and sometimes hang near the surface or the filter outflow. Gill-only infections can kill a fish before a single spot appears on the body, which is why laboured breathing with no visible cause deserves attention.

The four things people mistake for ich

Velvet

Velvet (Oodinium) produces a fine gold or rust-coloured dust rather than distinct white dots, and it’s much finer — more like powdered sugar than table salt. Shine a flashlight at the fish from the side in a dark room; velvet has a metallic sheen that ich doesn’t. It also moves much faster than ich and is more often fatal. If your fish are dying quickly with only a faint dusting, treat for velvet, not ich.

Fungus

True fungal infections look cottony and have visible texture — you can see filaments. They also tend to grow from a single point, usually an existing wound, rather than appearing all over. Ich spots are smooth and appear on healthy tissue.

Breeding tubercles

Male goldfish, koi, and some minnows develop small white bumps on the gill covers and leading edge of the pectoral fins during breeding condition. These are normal, they appear in a symmetrical pattern in those two specific locations, and the fish behaves completely normally. This one causes a lot of unnecessary medicating.

Lymphocystis

A viral condition producing white or grey cauliflower-like lumps, usually on fin edges. The lumps are larger and more irregular than ich spots, they develop slowly over weeks, and there is no effective medication — it typically resolves on its own once the fish’s conditions improve.

Why ich gets worse before it gets better

Understanding the life cycle explains almost every treatment failure.

The visible spot is the trophont — the parasite feeding under the skin. While it’s in there, it is largely protected from medication. Eventually it drops off the fish and becomes a tomont, which attaches to substrate or decor and divides, producing hundreds of free-swimming theronts. Those theronts must find a host within roughly 48 hours or they die.

Two consequences follow, and both matter:

  1. Medication only works on the free-swimming stage. You are not killing the spots you can see. You are killing the next generation.
  2. Spots increasing after you start treatment is normal. Each parasite that dropped off produced hundreds more. This is not the medication failing, and people who panic and switch products at this point restart the clock and make things worse.

The cycle is temperature-dependent — faster when warm, much slower when cold. At 78–80°F it runs roughly 5–7 days. Below 70°F it can stretch past two weeks, which is why cold-water treatment must run far longer than people expect.

How to treat it

Heat and salt

For a community tank of hardy tropical fish, raise the temperature gradually to 86°F over 24–48 hours and add aquarium salt at roughly 1 tablespoon per 5 gallons. Heat speeds the life cycle so parasites reach the vulnerable free-swimming stage faster; salt disrupts them at that stage.

Two cautions. Warm water holds less oxygen, so add an airstone or increase surface agitation — fish that survive ich and then suffocate is a real outcome. And scaleless fish are salt-sensitive: loaches, most catfish including corydoras, and similar species should get half the dose or a medication-based approach instead.

Medication

For severe outbreaks, cold-water tanks, or scaleless species, use a commercial ich treatment. Most contain malachite green, formalin, or a combination. Follow the label dosing exactly, and remove chemical filtration (carbon) during treatment or it will strip the medication straight back out.

The part everyone gets wrong

Continue treatment for at least one full week after the last visible spot disappears. The stages you can’t see are still cycling. Stopping when the fish look clean is the single most common reason ich comes back two weeks later, and reinfections tend to be worse than the original.

Vacuum the substrate during water changes throughout treatment — tomonts settle there, and physically removing them shortens the whole process.

Preventing the next outbreak

Ich doesn’t appear from nothing. It arrives on a new fish, plant, or a splash of water from a store bag. Then it takes hold when something has stressed the existing fish enough to lower their resistance — a temperature swing, a water quality problem, aggression, or a rough shipping experience.

The single highest-return habit is quarantining every new fish for 2–4 weeks in a separate tank. It prevents most disease introductions outright, and it’s cheaper than the medication for one outbreak.

Beyond that: keep temperature stable, keep ammonia and nitrite at zero, don’t overstock, and pay attention to behaviour. Flashing and clamped fins are early signals, and catching an outbreak in the first day or two makes it a minor event instead of a major one.

When to worry

Ich is very treatable when caught early. Escalate your urgency if you see heavy breathing with few visible spots (likely gill involvement), if fish are dying before spots develop (consider velvet), or if the spots come with red streaking, ulcers, or rapid deterioration — that combination suggests a secondary bacterial infection needing its own treatment.
